For more information about how to apply for extended benefits, see Information On Unemployment Insurance Extensions on the Division of Unemployment Assistance web site. You can file an extended benefit claim by calling the special toll-free number: 1-888-998-8418.
Straight from their web site.
Keep in mind, extensions are in the process of expiring, so you probably also want to check this chart: http://www.mass.gov/lwd/unemployment-insur/filing-a-claim/federal-extension-programs/overview-of-latest-extension-benefits.html which will tell you IF the extension you are looking for, is in force any more.
